I am a member and would like to facilitate a temporary pass for a friend:

  • CASH ONLY

  • Group text yourself, your friend, and 520-638-6227: “This is your name and I would like a temp pass for your friend. This pass is for (date range).”

  • Send a photograph of the front of friend’s drivers license.

  • You/your friend will be sent a link to forms for your friend to complete, including day pass and liability waiver

We are still limiting our memberships and our temporary passes. These are not designed for repeated use by the same guest. As this protocol changes in response to various benchmarks, this page will be updated accordingly. We will be more accommodating based on membership and geographic changes.

    • “This is friends name and I would like to join. I was referred by your name

    • We will send your friend a link to the initial form and schedule a time to meet

I am a member and would like to schedule a physical therapy appointment:

  • Call 520-977-1516 during standard business hours and Espi, our office manager, will schedule you at your preferred location based on availability

  • Call your insurance (the number is on your insurance card) and ask for an explanation of your benefits and what costs you are responsible for.

    • Cost of service, when utilizing insurance, is dependent upon your plan, deductible, and whether or not you’ve met that deductible.

    • CASH PAY is available and preferred.

  • Please do not ask for an evaluation in passing - there are far too many variables to address regarding “hurt knees” and “tight shoulders” and etc.

I am a member and would like to cancel or temporarily hold my membership:

  • Text 520-638-6227: “This is your name and I would like to cancel/hold my membership”

    • Holds max out at 6 months in our software and you will be responsible for tracking the billing and requesting an extension. If you need additional time, reach out prior to billing, as we are no longer refunding after 6 months.
